Clinical Observation on Bone Tumors


Abstract
Clinical observation were presented on 35 cases of bone tumors was admitted and treaed to the department of orthopaedic surgery, Pusan National University Hospital from January, 1958 to April 15, 1964 during about 6 years and 3 months.
1) 35 cases of bone tumors are classified as following;
Among benign bone tumors of 10 cases;
a. Osteochondroma 2 b. 0steoid Osteoma
1 c. Bone cyst
2 d. Giant cell tumor 5 Among malignant bone tumors of ¢¥25 cases; a. Osteogenic sarcoma 20
b. Fibrosarcoma 1
c. Ewing¢¥s sarcoma 1
d. Metastatic bone carcinoma 3
2) Among 25 cases of bone tumors, osteogenic sarcoma was 20 cases (57.1%) and benign giant cell tumor was 5 cases (14.2%). 0steogenic sarcoma was occupied above 50 percent of all bone tumors. Ratio between benign and malignant bone tumor was 10:25. Malignant bone tumor was abundant in twice than benign.
3) Age distribution of bone tumors was 10 to 40 in benign., 10 to 60 in malignant, and .33,5(average) in osteogenic sarcoma.
4) Ratio between male and female are 23:12,.in benign bone tumors 4:6, and in malignant bone tumors 19:6,.
5) Involvement of skeletal site was as following;
Involved skeletal site in 35 cases:
a. Femur 141
b. Humerus 7.
c. Tibia 6
d. Ilium 5
e. Mandible 1
f. Clavivle 1
g. Rib I
In benign bone tumors of 10 cases;
a. Femur 5
b. Tibia 2
c. Mandible 1
d. Humerus I
e. Ilium 1
In malignant bone tumors of 25 cases; a. Femur 9 b. Humerus 6 c. Ilium 4 d. Tibia 4 e. Clavicle 1 f. Rib 1
6) Geographical distribution of patients was 32 cases (91.4%) in Pusan and Kyong Sang Nam Do.
7) Among symptoms and signs, swelling or mass in 35 cases(100%), tenderness in 30 cases, limit of motion in 30 cases, pain sensation in 24 cases, and loss of weight in 21 cases. Especially malignant bone tumors was noted abundantly on general symptoms such as fever, fatigue, loss
of weight and dizziness."
8) Hematologically mild and moderate Eosinophilia was noted in all bone tumors and slightly anemic pattern. A/G ratio was 1:1, and slightly increased alkaline phosphatase was noted in malignant bone tumors.
9) Radiologically, cortical expansion with thining in 70%, and compact and differentiated bony outgrowth in 70% was noted in benign bone tumors.
Early 10 and rapid bony destruction in 76 % and rarefaction of bone in 76 % was noted in malignant bone tumors.
10) Among 10 cases of benign bone tumors, 1 cases received conservative treatment only, 6 cases received radical resection with curattage only, and remained 3 cases received radical resection with bone graft without operative mortality.
Among 25 cases of malignant bone tumors, 17 cases received conservative, chemotherapy and traction, 5 cases received amputation of limb, and 3 cases radical resection with bone graft without operative mortality. X-ray irradiation was not performed due to without establishment on P.N.U. Hospital.
11) Survival rate with following up studies was unsuccesful due. to uncertain of patient¢¥s address and uncooperation of patients.
